After logging into maps.google.co.uk and going to the “My Maps” tab there is a new facility to create personalised maps, save, share as a permanent link and even export as a KML file for viewing in the desktop Google Earth application.
If you are wanting to embed the Google Maps within a web page and go beyond the permanent link the KML file can be used to overlay onto a map invoked via the existing and extensive provided API.
A very quick and crude sample showing the "North East" of England region with two placemarks (one including an embedded YouTube video) is available here.
